OPHTHALMOLOGY

ANTERIOR UVEITIS

Inflammation confined to the iris(Iritis) and ciliary body (oxford)
Prompt
Response
Irregular pupil (due to synechiae)
Cells/flare in the anterior chamber or Keratic precipitates
Perilimbal conjunctival injection
Small hypopyon (Hypopyon = pus behind cornea, looks like white hyphema)
Cloudy cornea/central corneal opacity
Photophobia
Self-rate:
PromptExamination findings
ResponseVA - reduced due to central corneal opacification
IOP - Normal, can be raised due to synechiae
Slit lamp/fluorescein uptake on ulcer, raised corneal cells, flare in anterior chamber
Corneal scraping for MCS (? bacterial growth)
Self-rate:
PromptCauses of anterior uveitis
ResponseInflammatory
  • Inflammatory bowel disease (Crohns)
  • Inflammatory arthropathies (SLE, Sarcoid, Ankylosing spondylitis, psoriasis, reuters syndrome)
Traumatic
Infectious
Drug related
Self-rate:
PromptManagement of Anterior uveitis
ResponseOphthalmology consultation
Steroid - topical or systemic. eg:
  • Dexamethasone 0.1% drops - Q1h x 48h >> QID 7/7 >> TDS >>BD >> Cease
Cycloplegic drops - Homatropine 1 drop Q5 min until dilated then 1 drop BD (alternative tropicamide, cyclopentolate, Atropine 0.5% 1 drop BD) until anterior chamber reaction is under control
Analgesia (endone/paracetamol/ibuprofen)
IOP lowering agents (Acetazolamide, timolol)
Self-rate:
PromptComplications of anterior uveitis
ResponsePosterior synechiae
Glaucoma
Band keratopathy
pain
Visual loss
cataract
