Significant weight loss or gain, or change in appetite nearly every day
Slowing of thought or reduction of physical movement (observed by others)
Fatigue or loss of energy nearly every day
Worthlessness or excessive/inappropriate guilt nearly every day
Diminished ability to think/concentrate, indecisive nearly every day
Recurrent thoughts of death/suicide (+/- plan or attempt)
